Enrollment Procedure
When enrolling your child, you must submit:
–  Enrollment application
–  Medical form and immunization record
–  Emergency transportation authorization
–  Consent form for camera system
–  Non-refundable registration fee
–  CACFP Forms
–  Child Information Sheet
–  Authorized Pick Up
–  Van Transportation (SA only)
–  Verification of Center’s Procedures and Policies

Tuition Payment
Tuition payments, in the form of a check or money order, are due on Monday of the current week of service.  A late fee of $10.00 is assessed for accounts not paid by Wednesday 6:00 p.m. the week of service.  If payment with $10.00 late fee included is not made by Friday 6:00 p.m. the week of service, the child will be dis-enrolled unless you pay for both weeks the following Monday before taking your child to the classroom.

Returned checks due to insufficient funds will be charged a $35.00 service charge onto your next week’s tuition.  Arrangements can be made to resubmit the check or replace it with cash upon which time you will be given the returned check.  After the second occurrence, only a money order will be accepted.

Upon arrival, your child must be accompanied into the building.  As you enter, you will automatically be checked into the computer through our security system.  Please take your child to their assigned classroom and make the staff member aware of your child’s presence.  As you leave, please look at the parent bulletin board for daily events or new information.

At the time of departure, a parent must alert the teacher that they are leaving with their child.  Parents may be asked to show identification if the attending employee is unfamiliar with the parent.  If someone other than the parent or designated person is to pick up the child, the Center must be notified in writing, fax, or E-mail.  These individuals must present a photo I.D. in order for the child to be released.

Late Child Pick-Up
Parents who leave the child beyond the hours of our Center will be charged $2.00 for the first 5 minutes and $1.00 per minute after the time following 6:00 p.m.  This payment is payable to the employee responsible for their child and payable at time of pick-up.  If a parent realizes that circumstances beyond their control are going to delay pick-up, then a phone call should be made to the Center and the situation will be assessed as to the late payment.  Chronic late pick-ups could lead to dis-enrollment.

Full tuition payment is to be paid regardless of the child’s attendance.  This means that if your child is absent due to vacation, illness, etc., full tuition is to be paid as usual.  After attending DDLC for one continuous year, each family is eligible to receive a total of 2 “free weeks of tuition” per year (no yearly rollover), to be used consecutively or separately.  Those children who are on vacation for an extended period of time (i.e. summer vacation) may withdrawal for that time period.  A re-registration fee will be required to re-enroll the child at the end of the time period and must be paid at the time of withdrawal to guarantee an enrollment spot for the child.

We will be closed for the following holidays each year; New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas Day.  If a holiday falls on a Saturday the center will be closed Friday.  If a holiday falls on a Sunday, the center will be closed on Monday.  You are responsible for your regular weekly payment.  Tuition is based on enrollment not attendance.

Weather Emergencies/School Cancellation
When extreme weather conditions exist, the Center may close.  Closings will be announced on the radio stations K99.1 FM and 1290 AM, and on television Channel 7 WHIO TV.  Otherwise, the Center will be open the regular hours of operation.  There will be no tuition refund for emergency situations beyond our control.

Meals and Snacks
Every child that is at the Center between the hours of 6:30 a.m. and 8:30 a.m. will be given the opportunity to eat breakfast.  Lunch will be served to all children.  Each child will receive an afternoon snack.  A meal and snack menu is posted on the parent bulletin board.
